Reporting your incapacity for work

Report your incapacity for work to KBC Insurance
- Complete the first part of the ‘Incapacity for work declaration’ form
- Ask your doctor to complete the second part entitled ‘Medical certificate’
- Send the form to

  Consulting physician
  Prof. Roger Van Overstraetenplein 2, 3000 Leuven
- Or hand it in at your KBC Insurance agent
